Your Choose Aware form will have all your super fund details already filled in. Just log in, download the form and send it to your employer.

  1. Log in to Member Online
  2. Click Profile tab, select Changing jobs 
  3. Download form and send it to your employer.

Got the app? Email your super details in a few taps

Our app can write an email with your fund details and send it to your employer.

  1. Log in to the app
  2. Tap Profile then Email your employer
  3. Type in their email and press Send.

Take the work out of paperwork

Your pre-filled form has everything you need to tell your employer to pay contributions into your Aware Super account.

But if you need your details, here’s how to find them.

You'll find your member number by logging in to your Member Online account.

If you're using the Aware Super app, you'll find your member number in your digital member card on the app.

It's also on your annual statement, at the top right-hand side of the first page. Note that the member number is different to your account number.

Get help to log in

You’ll find your account number by logging in to Member Online and listed under ‘My Accounts’.

If you’re using the Aware Super app, you'll find your account number at the top of your dashboard above your account balance.

It doesn’t matter which form you choose, but the Choose Aware Super form comes with all the fund details pre-filled, including your name, member number and account number.

Or you can use your employer’s form, called the Super standard choice form.

But, you’ll need to fill it out with the Aware Super fund details.
